New Co-Management Plan Opt-in form

As communicated, Dr Geoff Kewley is retiring on 30 May 2026. Your co-management plan, established between Dr Kewley and your ADHD Lifespan clinic GP, will no longer remain valid beyond this date.

To assist you in ensuring continuity of care, one of our associated specialists will provide a dedicated 30 minute telehealth appointment for patients transitioning from Dr Kewley. This appointment will review your medical history and establish a new co-management plan, allowing ongoing ADHD care to continue seamlessly.

The consultation fee is approximately $300, and Medicare rebates apply for eligible patients.

Where possible, this specialist appointment will be scheduled in place of your next 6-monthly review. Your ADHD Lifespan GP will continue to provide your ADHD care once the new co-management plan is in place.

Please be assured that all relevant medical records, medication history, and an appropriate document from our clinic will be forwarded to the specialist in advance to support a smooth and efficient transition.
